Medical Record Requests
UPMC has a deep commitment to protecting the privacy of your medical information.
There are three main ways you can get access to your health records:
- Log in to MyUPMC. If you don't have an account yet, apply for MyUPMC online.
- Fill out a medical records release form.
- Contact your doctor or hospital.

Patient and patient-directed requests for medical records
Patients may request a copy of their medical record or ask us to send them to someone else.
To safeguard your privacy, you must complete and sign a protected health information (PHI) release form before we can send your records.
On the form, you can let us know:
- What records you want us to release.
- Where to send your records.
- What format — either electronic or paper — you want your records.
Doctors or others who want access to your medical records must also complete the HIPAA-compliant release form.
UPMC charges modest, cost-based fees for copies of medical records. We use the Pa. Department of Health's charges for medical records fee schedule.

Make Changes to Your Medical Record
Every UPMC patient can request a change to their health record.
To amend your record, complete the UPMC patient amendment to PHI form and mail it to the proper medical records department. The process may take up to 60 days.
Please note, we may consult your doctor before making changes to your record. UPMC can also deny the request if we deem your record correct and complete.
